Consumer Feedback Form in 20 languages

The Centre for Cultural Diversity in Ageing has developed a Consumer Feedback Form for use by aged care providers as an easy and accessible way for consumers, family members, representatives and others to provide feedback to the service. 

The form is in bilingual format and available in Arabic, Chinese (Simplified and Traditional), Croatian, Dutch, English, French, German, Greek, Indonesian, Italian, Macedonian, Maltese, Nepali, Polish, Portuguese, Russian, Sinhalese, Spanish, and Vietnamese.
